You could try the 60mm macro lens and ring light flash. A 100 mm macro lens might be overkill and bulkier to work with.
But try both in a camera store before you buy to see which gives you the effect you like best.
Typically macro lenses are very sensitive to motion and blur easily. So when you test (try the flash system too to see if the lighting is too blown out for you - the twin macro flash definitely would be, the ring should be ok) make sure you're able to get an exposure that's sharp enough with handheld use.
You will, of course, need cheek and lip retractors too. Which the patient can hold in place for you while you take desired shots.
Experiment to find the best position/angle to shoot from so you don't get glare on teeth or braces. But then stick to a consistent position for subsequent shots.
